Primary Veterinary Clinic

By providing Companion Protect with your primary veterinary clinic's contact information, Companion Protect will review your pet's medical history and your pet may not need an enrollment exam. If your pet does not have a primary veterinary clinic or medical history or has not been to the veterinarian in the past year, we will provide instructions for an enrollment exam.

I understand that my card will have a hold placed on it in the amount of the monthly premium and transaction fee. This hold will be cleared if the enrollment is not completed. If the enrollment is completed the card will be charged the first monthly premium and transaction fee. Ongoing monthly premium and transaction fees will be charged 5 days in advance of monthly policy period.
